By default, all users will sign the Player's Health Protect Promise waiver as a part of utilizing the risk management platform. Player's Health also allows the ability to incorporate your organizations unique waivers and consent forms for coaches to sign. What you need to know about this option:

• these should be documents that need to be read and signed to confirm user agreement
  • if your documents require text fields to be filled in, questions to be answered, etc. then this feature cannot yet accommodate that need
• this option requires the assistance of the Player's Health development team currently
  • partner with your Onboarding Manager or Account Manager to inquire further
  • please allow for 2-3 week lead time to set up
• basic formatting only
  • indent, bullets, numbers, roman numerals, etc.
  • no custom graphics or unique designs - see sample design and formatting below
